Position: Registered Dietitian

The Superior Family Health Team is seeking a Registered Dietitian who is a dynamic team player to join an innovative, community-based primary health care team.
Working as a Registered Dietitian on a multidisciplinary team, you will:
飪榃ork collaboratively with other Interdisciplinary Health Professionals to provide comprehensive care plans for our patients
飪楤e involved in health promotion and education for our patients and team members
飪楶rovide nutritional counselling and develop specialized nutrition therapy to address nutritional related illnesses
飪楥onduct nutrition assessments, develop nutrition care plans, and monitor effectiveness of the nutrition care plans through follow-up visits
飪楩acilitate group education sessions
飪楤e a key member of the diabetes and chronic disease management programs and support other Quality Improvement initiatives and programs
飪楤e a resource to family doctors and other allied health professionals
Your profile will include:
飪楤accalaureate degree in Dietetics
飪楳ember in good standing with the College of Dietitians of Ontario and proof of liability insurance
飪楢bility to work independently and collaboratively within an interdisciplinary health care team
飪楨xperience in individual counselling
飪楽uperior leadership, organization, research, evaluation, time management, communication and interpersonal skills
飪楨xhibit an ability to open and non-judgmental
